Athletes who intend to play a winter sport and do not have a current ImPACT baseline test should sign up for one of the dates below. There will be no other testing sessions available for winter sports.

FAQ’s

Who needs to take this?
Any athlete who has not yet taken the test for the 2015-2016 school year, except those who took the test last year.  If you have questions please contact your athletic trainer or the athletic office.

What is this for?
The ImPACT test is a baseline test of brain function that will be used if an athlete sustains a concussion to help determine if any brain function is impaired.

I took this test 2 years ago so why do I need to take it again?
Because brains in high school develop at such a quick rate it is recommended that athletes be tested every 2 years to ensure that the baseline test reflects where the brain is currently functioning.

How long will the test take and do I need to bring anything?
The test will take about 45 minutes start to finish. Parents to do NOT need to stay for the test and you do not need to bring anything with you.

If I go to East do I need to take the test at East?
No, all testing is done at West High School due to a larger availability of computer labs. During the test, each student will enter his or her school’s code to indicated which school they attend.

Where is room E-130 at West?
Room E-130 is located in the E2 Wing at West High School just past the Dux Pond (School Store).

Please be sure to choose a date and time. Testing sessions are subject to change if we don’t have any sign-ups for that particular day and time.

Zeeland Public Schools will be hosting our annual Pre-participation Physical Exam (PPE) Night on:

This serves as an affordable option for athletes to receive the annual pre-participation physical exam that is required to participate in athletics.  This PPE, however, is not intended to replace a more thorough annual physical provided by your primary-care physician for medical and insurance purposes.  The PPE is only intended to provide student-athletes an affordable option to meet MHSAA requirements for athletic participation.

All PPEs are $15 dollars.  CASH ONLY!!!  No health insurance of any kind is required to attend.  Athletes/parents must print the PPE form, attached below or found under the CHIX HQ tab under Zeeland Athletic Card (ZAC), and complete the first and last pages prior to coming to the event.  Parents do not need to stay if pages 1 and 3 are complete.  No appointment is necessary. However, time slots have been assigned based on last name to cut down on wait times.  Please try to come during your designated time or you may experience a longer wait.  The designated time slots are listed below.

If you are planning to play a winter sport and need to take your ImPACT test there are 6 testing dates for you to attend.  Failure to attend one of these dates will result in potential loss of playing time.  Athletes may attend a testing session at either school.  Spring athletes are also welcome to test at this time.

All athletes must have a valid baseline ImPACT test in order to receive their Zeeland Athletic Card (ZAC).

  • A valid test is one taken within the last two calendar years. Generally, all freshmen and juniors, as well as any students who have yet to play a sport at Zeeland East, must take the test. If you are not sure if you have a valid test please contact your schools athletic trainer for information.

Any ImPACT testing held after these dates will be conducted on an as needed basis. This may result it significant loss of practice/try-out time for those athletes still needing to take it, so it is in your best interest to make it to one of these testing times.

Multiple Teams · Everything you need to know to be ready for Fall Sports


Attention all Zeeland East and West  
Athletes and Parents:

Both Zeeland East and West Athletic Departments require ALL athletes to have a ZAC (Zeeland Athletic Card) in order to try-out for a sports team. Completed ZAC’s will then be given to your coach (by YOU) on the first day of tryouts or practice. Athletes without a ZAC will not be allowed to participate until they have one.

The ZAC consists of the following items that must be completed prior to tryouts:
• An annual sports physical/health screening (on or after April 15, 2015).
• ImPACT testing (every 2 years)—All freshmen, juniors and athletes who did not take it in the 2014-2015 school year.
• Athletic “Understanding Concussions” Form—this form only needs to be filled out once during your high school career and must be signed by a parent and the athlete.
• Academic Eligibility-checked by the athletic departments each season-no form necessary.
• Athletic Code Violations-checked by the athletic departments each season-no form necessary.

When do I need to have my ZAC card to play fall sports?
• Football starts on Monday, August 10
• All other Fall sports begin on Wednesday, August 12

How do you get your ZAC?
Stop by your home school’s Athletic Office:
• Aug 3rd – 6th (Monday thru Thursday) from 9am-3pm
• Aug 10th & 11th from 9am-3pm
• Football players will have the opportunity to get their ZAC during football camp the week of July 27-30 if they have completed all the required components. Please see the East athletic trainer, Jason Burgess, prior to the beginning of football activities each day to receive you ZAC.

How do I complete my ImPACT testing?

Fall athletes should plan to have their ImPACT test completed by August 5th.  Very limited testing times will be offered prior to the winter and spring athletic seasons, so please make every attempt to attend one of our summer sessions. Please note that there will be NO ImPACT testing sessions August 6 & 7.

Where can I get an “Understanding Concussions” Form?
Concussion awareness forms are available on either school’s sports websites, as listed above, under CHIX HQ or DUX HQ in the “Forms” tab. The completed signature page must submitted to the East Athletic Office to be considered valid.

Where can I get a Physical Screening/Physical Form?
Physical screening forms are also available on either school’s sports websites in the “Forms” tab.

What do I do with my ZAC once I have it?
Turn it in to your coach on the first day of practice or tryouts. If you do not have one to hand to your coach you will not be allowed to participate until you do give one to your coach.

What if I don’t play a fall sport, do I still need to get my ZAC?
Zeeland Athletic Cards will be required for all seasons. It is recommended that you complete your ImPACT testing in the summer, complete your “Understanding Concussions” form and physical form as soon as possible. However, if you are not playing a fall sport, the card will be kept until your season as academic eligibility and code violations will need to be verified each season.

What if I get my card for a fall sport and decide not to play?
All cards have an expiration date and if not used by that date they will be invalid. Cards can be turned back in to the athletic department.

What if I lost my card?
Please see your home school athletic department to obtain a new card. There is a $1 replacement fee for lost cards.

All students must obtain a ZAC card before being allowed to try out, even if you played a fall sport!  You must present your card to the coach on the first day of tryouts.  Here are the items that must be completed before the card can be given.

Students may come into the athletic office any time it is open to obtain their ZAC card and to ask questions or find out what they still need.  Parents do not need to be there for students to obtain their card.  Please wait until about 2 weeks before your sport begins to obtain your card.  Student-Athletes, please remember it is your responsibility once you have your card to get it to your coach on the first day of try-outs.
